In Yoshi's Island, almost all levels have an elaborate color gradient set as the furthest background layer - including those where it is never visible due to other layers covering it. Here are examples of the levels as they normally appear and the hidden gradients in them.

In Mario Kart 7, the Lakitu driver's right hand is always open, while his left hand is always a fist. This can be seen in-game during his hurt animation (top). The reason for this is that he reuses the NPC Lakitu model, whose left hand is closed to hold the fishing rod (bottom).

In Paper Mario: The Thousand-Year Door, Koops normally has four fingers on each hand (left). However, during his Appeal animation, he briefly gains a fifth finger on his right hand (right). Note that his other hand still has only four fingers.

In the majority of 3D Mario games, Mario and Luigi's hat and hair are mutually exclusive, interchangeable parts of their models, never used together in the same scene. However, in Luigi's Mansion, there is a hair layer present underneath Luigi's hat in his main model.

In Mario Kart: Super Circuit, whenever Yoshi wins a race and waves towards the camera, a single dark pixel can be seen above Yoshi for exactly 5 frames (top). This pixel is in fact part of Yoshi's animation, which can be seen in sprite data extracted from the files (bottom).

In Donkey Kong 64, if a boss room is entered with the wrong Kong, the pre-boss cutscene will apply the intended Kong's animation data to the wrong Kong's model, often leading to distortions. The Pufftoss cutscene causes the chins of all non-Lanky Kong characters to be enlarged.

Whenever Blooper Babies appear in official artwork, they are depicted with eyes identical to adult Bloopers. In-game, they are very small, making it difficult to tell, but zooming in on them or extracting their models shows that they have blank white eyes with no pupils instead.
