In Super Mario 64, an oversight in the code results in the texture for one of the Scuttlebug enemy's eyes being mirrored along the wrong axis, giving it a lopsided appearance (left). By loading the model correctly, we can see what the eyes were intended to look like (right).

In Mario Kart Wii, the Battle menu shows two balloons and a coin. However, there are actually three balloons; one is simply 1/100 the size of the others. This is due to balloons always coming in threes and the developers deciding that shrinking it was easier than deleting it.

Super Mario 64 contains unused code for throwing MIPS the rabbit, including a unique animation of him falling onto the floor and getting back up. In the finished version of the game, this was replaced by Mario carefully setting MIPS down instead of throwing him.

If Tubba Blubba catches Mario while chasing him in Paper Mario, Mario is supposed to flee. However, using Bow's Outta Sight move will confuse Tubba Blubba, making him leave the battle. An even more obscure interaction is that rubbing Repel Gel on Mario achieves the same effect.

In Super Mario Galaxy 2, if Mario or Luigi are riding Yoshi and enter first-person view, there will be a single frame after returning from first-person view where their hands and most of their faces are not rendered.
